The WASH-Nutrition Nexus: How Clean Water and Sanitation Eliminate Child Stunting

The Stunting Crisis & Child Health Metrics

Child stunting (low height-for-age) affects 28% of children under five in Bangladesh. Stunting causes irreversible physical and cognitive deficits, impacting lifelong learning capacity and economic productivity.

Traditional nutritional supplementation programs often yield disappointing results because food intake alone cannot resolve underlying gut inflammation caused by poor sanitation.

Medical Mechanics of Environmental Enteropathy

The biological pathway linking poor WASH to stunting is Environmental Enteropathy (EE) — a chronic, subclinical inflammatory disorder of the small intestine caused by ingestion of faecal pathogens in unwashed hands and unsafe water.

Repeated pathogen exposure flattens intestinal villi, increases gut permeability ("leaky gut"), and diverts vital calories away from physical growth toward immune defense against intestinal infections.

The Faecal-Oral Transmission Pathways in Infants

Infants exploring their environment ingest faecal bacteria through multiple unhygienic vectors:

  • Crawling on earthen floors contaminated by domestic poultry faeces.
  • Drinking water stored in unwashed wide-mouthed clay pots.
  • Eating complementary foods prepared with unwashed hands.

Integrated WASH & Nutrition Action Plans

Eliminating Environmental Enteropathy requires integrating safe water points and handwashing stations directly into infant nutrition programs:

  • Pathogen-Free Water Supply: Installing AIDE Deep-Set Tubewells to supply pathogen-free water for food preparation and infant feeding.
  • Clean Play Zones: Constructing elevated, washable play pens to prevent infants from crawling on soil contaminated with poultry faeces.
  • Handwashing Coaching: Establishing handwashing stations with soap at primary feeding sites and latrine exits.

Community Mothers' Clubs & Clean Play Zones

AIDE forms village-level Mothers' Hygiene Clubs, conducting practical demonstrations on safe food storage, water chlorination, and infant play area sanitation.

Combining clean water infrastructure with maternal hygiene coaching stops gut inflammation and restores healthy child growth trajectory.
